INDICATIONS: Oedema of hepatic, cardiac or renal disease. Essential hypertension, diabetes insipidus, idopethic hypercalciuria.
CONTRA INDICATIONS: Toxaemia of pregnancy.Renal decompensation; anuria
DOSE
Adults
Oedema: Initially 25-100mg daily in divided doses,reduced for maintenance if possible; severe oedema in patients unable to tolerate loop diuretics, initially 75mg daily. Hypertension: 12.5-25mg daily in divided doses, increased to 50mg daily if necessary.
Elderly
In some patients an initial dose of 12.5mg daily may be sufficient.
Children
Under 6 month; upto 3.5mg/kg daily, 6 month-2 years; 12.5-37.5mg daily, 2-12 years; 37.5-100mg daily, all in divided doses.
PRECAUTIONS: Check electrolyte levels periodically and electrolyte imbalance should be corrected. Care should be taken of hypokalemia when digoxin is being given concomitantly, cardiac arrhythmias. Hepatic or renal impairment, diabetes, gout.
Breast-feeding: Use with caution. Drug passes in breast milk. Old age: reduced dose.
SIDE EFFECTS: Hypokalemia,
weakness or fatigue, hyper-uricemia. Hyponatremia, skin rash, nausea, anorexia,
vomiting, epigastric discomfort, headache, paresthesias, hyperuricemia, fatigue
and blood dyscrasias. May induce hyperglycemia. Photosensitivity,
hypomagnesemia, hypercalcemia. INTERACTION:
Drugs
Cholestyramine & colestipol reduce absorption of thiazides, they antagonise the hypoglycemic effects of antidiabetics, increased risk of hypokalamia if used with corticosteroids.
